Care Coordinator II/Permanency Specialist

Hillside Children’s Center is hiring a Full Time Care Coordinator II/Permanency Specialist at our Varick Campus in Romulus, NY. This position is 40 hours per week, 9am-5pm with some evening and weekend work required.

The Care Coordinator II (CC II) Permanency Specialist is responsible for providing Family Finding and/or Child-Focused Adoption Recruitment for youth in out-of-home placement. The permanency specialist will work in partnership with Hillside Therapeutic Foster Care and/or Residential teams as well as County case workers. They will facilitate a lifetime network of support of biological and/or adoptive family members and natural supports who care for the youth. Additionally, the permanency specialist will support Permanency Round Table activities whenever possible. This position works with clinicians, care coordinators, case workers, funders and other care providers to create permanency for the youth.

Responsibilities include, but are not limited to:
• Working in collaboration with the youth's team members, family and natural supports in the provision of permanency services and the development of a lifetime support network and permanency plan for each youth.
• Actively involved in the implementation, monitoring evaluation and follow-up of a strength-based, family-focused lifetime network plan, including development of a Plan A-B-C for each youth, recognizing gaps and needs and ensuring that the plan is successfully implemented.
• Work in partnership with funders and staff in the county department of social services, providing on-going communication regarding permanency work with identified youth.
• Facilitate family meetings with large groups of family members and natural supports
• Facilitate and/or participate in Permanency Round Tables for youth enrolled in Hillside services
• Be responsible for meeting agency driving standards. Some out of town travel will be required to facilitate meetings with youth, family members, funders and others.
• Utilizes experience with Family Finding, child-focused recruitment, education, age-specific knowledge and cultural competence in order to assess and support children, youth and families to achieve positive permanency outcomes.

Qualifications

 

Bachelor’s Degree in Social Work, Psychology, Sociology or related Human Services field and 3-5 years of related experience is required. Master’s Degree in Social Work, Psychology, Sociology or related Human Services field and 5 or more years of experience is preferred. Candidates must have a valid NYS Driver’s License, and must meet agency driving requirements.
