Ambulatory RN II Transplant

Position Summary:

With a focus on renal transplant and hepatobiliary patients, this AMB RN is a member of a multidisciplinary team thatt provides advanced professional nursing care to those in varying states of health and illness through assessment, planning, implementation and evaluation of the patient's health needs. Daily responsibilities involve direct communication with patients and providers related to clinic appointments, ongoing surveillance of health status through labs and procedure results, and coordinating follow up as needed. This individual provides education to potential transplant recipients and living donors. S/he will attend hospital rounds and work closely with newly transplanted patients on all aspects of preparing for a safe discharge. This position works closely with surgeons and nephrologists as well as specialty pharmacists, dieticians and social workers.

Organizational Standards of Patient Care and Nursing Practice guide the delivery of nursing services by this individual. The position demonstrates dedication to professional growth and development and to the continuous improvement of patient care.

The individual in this role spends significant time on the computer and phone, and takes call one week a month.

Education:

Licensed RN in the State of Vermont.

Experience:

1 year  experience in relevant clinical area and/or independent in all areas of the Ambulatory RN I accountabilities required. Experience in Transplant, Dialysis or Nephrology preferred.
